      I, Efstathios Vlachos, aged 30 years, occupation bartender, do declare on oath that my personal description is: color white, complexion dark, height 5 feet 6 inches, weight 185 pounds, color of hair brown, color of eyes brown, other visible distinctive marks none.
      I was born in St. John, Greece on the 25 day of September 1878; I now reside at Athens Hotel Coney island. I emigrated to the US from Havre France on the vessel Bismark; my last foreign residence was St. John Greece. It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state, or sovereignty, and particularly to George I King of Greece of which I am not a subject; I arrived at the port of New York on or about the 10 day of August 1897...

      To the Honorable the District Court of the United States for the Eastern District of New York:
      The petition of Efstathios Vlachos hereby filed, respectfully showeth:
      First. My place of residence is Brooklyn House, Bowery, Coney Island
      Second. My occupation is bartender
      Third. I was born on the 25 day of September 1878 at St. Johns, Greece
      Fourth. I emigrated to the US from Havre, France, on or about the 3 day of August 1897 and arrived in the US at the port of New York on the 10 day of August 1897 on the vessel Bismark.
      Fifth. I declared my intention to become a citizen of the US on the 9 day of November 1908 at Supreme Court of the County of Kings.
      Sixth. I am not married. I have no children.
      Seventh. ...renounce sovereignty to George T King of the Greeks...
      Eighth. I am able to speak the English language.
      Ninth. I have resided continuously in the US for the term of five years at least...since the 10 day of August 1897 and in the State of NY...since the 10 day of August 1897.
      Tenth. I have not heretofore made petition for citizenship to any court.
